In order for this program to be effective in helping solve crime, we kindly request that you provide us with the following information, if possible (please use the below bullet points as a checklist in your submission of details):
- The name of a suspect/suspects (real name or slang name);
- The race of the suspect/suspects;
- Suspect's address or hang outs;
- Suspect's vehicle tag number with color and make of the vehicle;
- The name of the victim;
- The race of the victim;
- The location in which the incident took place (the city and state) and the location of the victim (dead or alive);
- The date of the incident (if you do not know the exact date an approximation is sufficient);
If you are missing any of the above information, please provide us with as much information as possible and we will pass this information on to the appropriate law enforcement agency.
In order for you to recollect the incident in as much detail as possible, we recommend that you type all of the details regarding the incident out. Once completed, we recommend that you re-read the document to ensure it is how you remember the incident to have taken place.

Once completed, please click on the SUBMIT button and your information will be sent directly to Soldiers-And-Kids. Your submission is guaranteed to remain completely anonymous. Once we receive your information, we will foreword it to the appropriate law enforcement agency. None of your personal information, such as your name, will be attached. The case information will go directly to the law enforcement agency that has investigative authority, to ensure that a thorough investigation can be carried out.
